Job Summary

Baptist Health is looking to add a Care Coordination Educator at Baptist Medical Center Metro Square. This is a Full Time Day shift opportunity working Monday - Friday. The role of the Care Coordination Educator will be providing education to ambulatory care coordination nurses and patients in the primary care setting, developing and implement training materials, mentor staff, and identify resources to support the education of both team members and patients. Additionally, the position will play a key role in maintaining staff competencies, supporting workflow process improvements, and disseminating information to care coordination teams and stakeholders.

As a Care Coordination Educator, you will be responsible for:

  • Providing education to ambulatory care coordination nurses and patients in the primary care setting, while working collaboratively with the healthcare team to coordinate care..
  • Develop and implement training materials, mentor staff, and identify resources to support the education of both team members and patients.
  • Maintaining staff competencies, supporting workflow process improvements, and disseminating information to care coordination teams and stakeholders

Education Required

Master's Degree - Nursing

Education Preferred

Doctor of Nursing Practice (DNP)

Experience

  • Over 5 years Nursing Experience

License and Certification

  • Basic Cardiac Life Support (BCLS) Required
  • Licensed Registered Nurse Required
