Job Summary
Baptist Health is looking to add a Social Worker II to our Social Services team at Baptist Medical Center located in Jacksonville, FL. This is a PRN opportunity working primarily weekend shifts as they are needed within the department. As a Social Worker with Baptist your primary goal is to enhance patient quality outcomes while maintaining ongoing communication with the patient, family, medical staff, any caregivers or third party payors to set priorities, plan & organized, and implementation of care to decrease the amount of readmissions.
As a Social Worker II, you will be responsible for:
- High level screening, assessing and planning for an identified high risk group of patients in the Emergency Department.
- Collaborating with the attending and resident staff as well as nursing, ancillary, professional, and technical staff, to coordinate the delivery of high quality, individualized, cost effective patient care.
- Utilizing Utilization Review criteria along with admission and discharge criteria to appropriately screen patients for admission or discharge.
Education requirement: A Master's in Social Work is required (MSW)
