Job Summary

Baptist HealthCase Manager is looking for a Registered Nurse, at Baptist Medical Jacksonville. This is a Full-Time Days position in the In-Patient Case Management Department at Baptist Jacksonville.

As a Case Manager, you are responsible for:

• Care coordination for hospitalized patients.
• High risk screenings and assessments.
• Coordinating post acute care services with proper documentation.
• Collaborating with multidisciplinary team members.

Education Required

Associate's Degree - Nursing

Education Preferred

Bachelor's Degree - Nursing

Experience

• 1 year Case Management Experience

License and Certification

• Cardiopulmonary Resuscitation Certification (CPR) Required
• Licensed Registered Nurse Required
