Resource Navigator, The Players Center, Wolfson Children's Hospital, Baptist Downtown

Job Summary

Wolfson Children's Hospital is hiring a Resource Navigator to join The Players Center team.   This is a full-time, in-person position located on the downtown Jacksonville campus of Baptist Health.

Responsibilities include:

• Provide assistance to patients and families by screening for social determinants of health
• Identify opportunities to improve health
• Connect families to appropriate resources
• Follow-up to confirm the connections were made and beneficial
• Assist families in enrolling children into Medicaid, Children's Health Insurance Programs), and other public benefits.

**Case management or care coordination experience preferred.

**This position requires attendance at evening events (once or twice a year) and weekend events (approximately 2 weekends per month).
