Job Summary
Baptist AgeWell is seeking a Full-time Nurse Practitioner to join our Homecare team . The provider must be comfortable and have experience working with adults with chronic medical conditions in a homecare setting. Proficiency with EMR required. Excellent oral and written communication skills.

License and Certification
- DEA Controlled Substances Registration Required
- Certified Nurse Practitioner Required
- Family Nurse Practitioner (FNP) Preferred
- Adult-Gerontology Acute Care Nurse Practitioner (AGACNP) Preferred
- Adult-Gerontology Nurse Practitioner (AGNP) Preferred
- Psychiatric-Mental health Nurse Practitioner (PMHNP) Preferred
- Licensed Advanced Registered Nurse Practitioner Required
- Autonomous Advanced Practice Registered Nurse Preferred
