Full-time, outpatient position
Minimal call responsibilities
Lead your care team, collaborating with experienced Advanced Practice Providers
Support from an extensive in-practice team consisting of behavioral health, social work, pharmacy, nutrition, care coordination, and rehab resources.
Locally governed, physician led, professionally managed and community focused program ensures autonomy and a voice in your practice
Epic EHR
M to F 8 am to 5pm
On call 2 to 3 weeks per year for Monday through Sunday.

What You Will Need
MD or DO
Active/unrestricted medical license
Board certified/eligible
Family Medicine or Internal Medicine trained with a passion for senior care and population health.
Education Required
Doctorate of Medicine
Education Preferred
Doctorate of Medicine
Experience
- Over 5 years Family Practice Experience Preferred
- Experience working with geriatric population Preferred
License and Certification
- Basic Life Support (BLS) Preferred
- Licensed Physician Required
- Drug Enforcement Administration (DEA) License Required
