Helping You Navigate All the Layers of Care
We’re not your typical doctor’s office. Let us serve as the point person to help you coordinate care among the many needed services and specialists. This approach of having one set of eyes on all aspects of care provides better results than the fragmented care that’s typical among older adults with complex needs.
Longer Appointments
We won’t rush you. Our doctors take the time to listen and ensure you're getting the comprehensive care you need.
Specialized Team
Our team of doctors, pharmacists, rehab therapists and mental health experts specialize in senior care with the united goal of keeping you healthy.
Complete Health Picture
The pillar of our program is the initial comprehensive assessment, which provides a complete picture of your total health. Now we can gauge where you are and help you make choices to ensure you stay active and are able to do the things you love.
Accepts Medicare
Your personalized care at AgeWell Center is covered by Medicare.
House Calls
Patients with significant limitations that make visits to a doctor’s office difficult may be able to get basic primary care services at home.

Seniors Have Unique Health Needs
Our specialized programs tailor care specifically for those needs.
-
InBalance
Get an evaluation to establish a baseline of your balance. Our team works together to develop a customized conditioning schedule to help you stay strong, stable on your feet, and maintain mobility so you can keep up with the grandkids.
-
InConfidence
You don’t have to accept bladder control problems as just a part of life. Many incontinence issues can be improved by different therapies offered in our InConfidence program. Pelvic muscle exercises, behavioral therapy, biofeedback or medication can make a big difference in your life.
-
Mind Your Mind
Occasional forgetfulness can happen to anyone, but it doesn’t have to be a sign of aging. For some, adjustments to medication, better nutrition and more sleep can improve memory. Others may benefit from a cognitive assessment and healthy lifestyle changes.
