Job Summary

Baptist Medical Center is looking to add a Clinical Triage and Referral Coordinator to our Outpatient Behavioral Health department. This is a Full-Time Day shift opportunity working in office for at least 90 days and if 90 days is completed without issue, candidate has option to move remote.

As a Triage and Referral Coordinator, you will be part of the collaborative care team which includes, but is not limited to, various mental health professionals, psychiatrists, and primary care physicians. The team will be utilizing the collaborative care model (C0CM) to assist with mental health needs within a primary care setting.

This position functions at the very center of the the Outpatient Behavioral Health department and will be part of the Collaborative Care Team (CoCM). Using exceptionally well-developed, strong verbal, listening and written communication, coupled with the expertise of administering evidenced screeners and mini mental status exams, this role facilitates triage and screening of all referrals to the outpatient behavioral health department. This work includes, but is not limited to, active listening, independent clinical judgement, and patient engagement to attain appropriate information to determine the most appropriate type of behavioral health services needed. The coordinator explains the services recommended and appropriately links patient in a timely fashion. Administratively the coordinator will assist with scheduling and phone coverage for the clinical team

**This role requires a Bachelor's Degree in Psychology or related field.
