Job Summary

We're hiring a Referrals Coordinator for the AgeWell Department in Jacksonville, Florida. This is a full-time days position located at Baptist AgeWell Downtown Jacksonville.

The AgeWell Department is seeking a Referrals Coordinator to support their team. The Referrals Coordinator will interact with staff, patients, internal departments, physician practices and insurance companies to obtain maximum reimbursement through established authorization procedures and order entry. This position requires a candidate who has previous referrals and healthcare experience as well as excellent customer service skills.

Shift Details: Monday-Friday 8:00am - 5:00pm.

Referrals Coordinator, Baptist AgeWell Department, Job Responsibilities: 

  

  • Multi-tasking and being highly organized.
  • Serving as an advocate for the patient and the patient's family.
  • Assisting patients with obtaining oxygen, power mobility devices, diabetic shoes, orthotics and other medical equipment covered by Medicare.
  • Tracking referrals to avoid duplications, to ensure completion, and to verify documentation is in EMR.
  • Processing pre-authorizations and travel accommodations for rehabilitation appointments.
  • Processing patient referrals to external services (e.g. cardiology, gastroenterology) and internal services (e.g. AgeWell PT, OT, SLP, Pharmacy, SS, Bladder Health) for an at-risk patient population (frail elderly, cognitive deficits).
  • Accurately completing verification of insurance benefits and authorizations using online tools and/or direct contact with insurance carrier.
  • Registering patients in a timely, accurate, and professional manner which reflects the core values of Baptist Health.
  • Receiving, distributing and/or processing faxes relating to the care of the patient.
