Thank you for choosing Lyerly Neurosurgery. Our phones are open from 8am to 4:45pm every business day (except holidays) and we are happy to answer your questions. Our physicians are on call 24/7 for emergencies. If you should call us with an urgent request and need us to call you back, please be sure you are accessible and have not blocked incoming calls on your phone.

Arriving for Your Appointment

We want to make your experience with us a positive one, and we know that parking can sometimes be a challenge, so please plan to arrive on campus and park 45 minutes before your appointment so you don’t feel rushed. That will give you sufficient time to arrive at our office 30 minutes prior to your appointment time to complete all necessary forms in the office.

What You Will Need to Bring

If you have received our patient packet by mail, please complete all pre-registration forms and bring them with you to your appointment. In addition, please bring any related medical records, imaging films or disks that have been requested. You will need your driver’s license, insurance card(s), and co-pay.

Directions and Parking

Our main office is located on the 11th floor of the Weaver Tower on the campus of Baptist Medical Center Jacksonville. Park in the garage labeled P2.

Directions from the North: Take I-95 South across the Fuller Warren Bridge. Exit on San Marco Blvd., turn left at the first signal (Palm Ave.), enter P2 parking garage on the left.

Directions from the South/Beaches: Take Atlantic Blvd. (US 10) or Beach Blvd. (US 90) to I-95 North. Exit on Prudential Dr., turn left. Enter P2 parking garage on the right.

Directions from the West: Take I-10 East to I-95 South across the Fuller Warren Bridge. Exit on San Marco Blvd., turn left at the first signal (Palm Ave.), enter P2 parking garage on the left.

Appointment Cancellation

Please notify us at least 24 hours in advance if you find it necessary to cancel an appointment. This gives us the opportunity to schedule another patient at that time.

Answering Your Questions — 24/7

Each of our neurosurgeons also has a dedicated team of clinical staff available to answer your questions. We're open Monday through Friday from 8am to 4:45pm, but a physician is always on call for emergencies. Due to the rotation of this "on call" schedule, you may occasionally be seen by another physician in the group. Occasionally the physicians are behind schedule due to emergencies. You will be notified if your appointment must be rescheduled. We request your understanding and cooperation if this should happen.

Please note: The physicians will not prescribe pain medication at night or on the weekend. If you have a block on your telephone, please remove it before paging the doctors.
