Grievance Process

  • In accordance with Florida Statute 395.301, the Baptist Health Grievance Process provides an individual the opportunity to dispute charges that appear on a patient’s itemized statement or bill.
  • We recommend you first raise any concerns about charges on an itemized statement or bill with billing customer service, as they are often able to answer common questions and concerns. Then, if you wish to further dispute those charges, please complete this form below, or you can print it and mail to: Baptist Health Written Grievance Process, 3563 Philips Highway, Suite 201B, Jacksonville, FL 32207.
