Streamlining Behavioral Health Access
The Collaborative Care Model (CoCM) makes it easier for patients to access behavioral health services through Baptist Primary Care. Baptist Primary Care providers team up with Baptist Behavioral Health providers so you can receive the help you need more quickly.
This collaboration between your primary care doctor and a dedicated Baptist Health licensed mental health provider ensures you get the resources and treatment you need without any long waits.
If you or your child is experiencing mild to moderate mental health concerns, such as depression or anxiety, your Baptist Primary Care physician can easily refer you to these services.

How does it work?
- Schedule an appointment with your primary care doctor to discuss your symptoms or concerns.
- Your primary doctor will determine if you need behavioral health services and refer you to a behavioral health care coordinator, a licensed therapist who works with a psychiatrist to treat mild to moderate psychiatric needs.
- Within 72 hours of referral, a care coordinator will contact you to schedule an initial assessment.
- Following your assessment, the care coordinator will confer with a consulting psychiatrist to discuss a treatment plan and other recommendations, such as medication management.
- The recommendations are then reviewed by your primary doctor, who will discuss the plan with you and prescribe any medications.
- The care coordinator will keep in regular contact with you and monitor your progress, sharing notes and recommendations with your primary doctor along the way.

What are the benefits?
Behavioral health services through the Collaborative Care Model (CoCM) are available for pediatric, adult, and geriatric patients.
- You'll get faster access to behavioral health services, and you essentially receive two services in one visit.
- You'll have access to both behavioral therapy and behavioral medications, if appropriate to your symptoms.
- Your primary care physician remains in control of your care and is kept up-to-date on your progress.
- You can choose to receive your behavioral health services in person, virtually, or over the phone.
- You pay only one monthly co-pay for your behavioral health collaborative care services.
